What type of communication is acceptable for meeting the requirements at DSCT.4? Can the nurse give the discharge papers to the patient and tell them to take this to the health care provider when they go for their visit?

The patient needs to be informed of the A1C result. This can be accomplished by including the A1C result on the Discharge papers given to the patient. The intent of this standard is that the inpatient health care practitioner communicates directly with the outpatient health care practitioner managing the diabetes. This can be accomplished by including the A1C result and any unresolved glycemic management issues, in communication to the outpatient health care practitioner. The discharge summary may be shared with outpatient health care practitioners by the following methods: sending or mailing, emailing, faxing, or a phone call providing the required information.
