When it comes to The Joint Commission’s Primary Care Medical Home (PCMH) Certification, Sand Point Community Health Center really took the model to heart. Sand Point CHC adopted the PCMH model about three years ago as a primary care delivery site of Eastern Aleutian Tribes, a Federally Qualified Health Center supported by a grant from the federal Health Resources and Services Administration.
