Stage 1: Normal or Easy Difficulty Playthrough, All Collectibles and Miscellaneous Trophies
It's highly recommended to play your first playthrough on either Easy or Normal difficulty in order to unlock Madhouse difficulty. Even if you got the Recovery Set Pack and Survival Handgun Set Pack DLCs as a pre-order bonus which automatically unlocks Madhouse difficulty, it's still recommended to start on an easier difficulty in order to get familiar with the game and to unlock extras you can use during your Madhouse difficulty playthrough. Keep in mind that only playing on normal will unlock the item The Secrets of Defense, which will come in handy on your Madhouse playthrough, so it is recommended to play on normal on either your first or your second run.
It's important to obtain all collectibles during this playthrough or else you will end up having to get them on your speedrun (which is perfectly possible, but avoidable) or you'll have to do an extra playthrough. The reason for this is that Madhouse difficulty has its own set of collectibles and during your speedrun you will have to focus on other trophies. Refer to the Mr. Nowhere trophy for a detailed and chronological collectible walkthrough with minimum spoilers.
During your first playthrough you will obviously obtain all the story-related trophies. There are 2 different endings in the game. It doesn't really matter which ending you pick first, as it doesn't have huge consequences gameplay wise. Another thing you want to keep in mind during this playthrough are the miscellaneous trophies. Most of these trophies are combat related or require you to do specific tasks, making them all missable. Below are the trophies in questions listed in chronological order:

If you happen to miss any of these trophies, it's not really that big of a deal. Any of these trophies can easily be acquired during your speedrun or even during your madhouse difficulty playthrough. If you're trying to avoid as many spoilers as you can or you're scared you might miss one of these trophies, it's recommended to make several separate save files. This will make sure you won't have to replay the entire game if you happen to miss any of these miscellaneous trophies.
Stage 2: Sub 4 Hours Speedrun, Max 3 Item Box Uses and Max 3 First Aid Meds Uses
While it might sound a little odd to you to do the speedrun trophy on your second playthrough, doing so will make your Madhouse difficulty playthrough a lot easier. Beating the game in under 4 hours doesn't just net you a trophy, it also unlocks the Circular Saw as a weapon. The Circular Saw is a powerful melee weapon that doesn't require any ammunition and will do lots of damage to bosses on your Madhouse difficulty playthrough. That being said, now that you've got every collectible in the game you should be somewhat familiar with the maps. The speedrun trophy requires you to beat the game in under 4 hours and that is a fairly forgiving requirement. The game can easily be beaten in around 1:40 hours if you don't make any mistakes, just like most old-school Resident Evil games. Of course you are not aiming for a top-notch run like that, but around 2:30 to 3 hours should be a very plausible time for your second playthrough if you try to speedrun it. One thing you can keep in mind is that you can abuse the save system and make multiple save files in case something goes horribly wrong and you want to redo a certain section. Simply restarting from the in-game menu and of course dying will still add to your time, so you will have to go all the way back to the main menu and load your save file. You can pause your game and your inactivity won't add towards your time! This will be helpful if you are following a speedrun walkthrough at the same time.
During this playthrough, you will also want to go for the Resource Manager and Walk It Off trophies. These trophies require to limit the usage of item boxes and first aid medicines. This won't be a huge problem, since you will be running through the game and avoiding enemies for most of the time anyway. For the item box related trophy, there are actually 2 spots in the game where you absolutely have to make use of an item box:
You will need to empty your inventory before you can enter Lucas' Happy Birthday room.
You will need to pick up your items again after the room (do not forget to bring the crank!) to proceed in the main story.
(Optional) After you escape the old ship, you can pick up your items again for the final enemy encounters and the final boss. A work-around for the final boss is to make use of the 'Manage Items' feature that will appear once you die on a boss fight. However, the downside of this is having to get to the final boss without any weapons. This is manageable on easy though.
For the first aid medicines related trophy, you can actually use steroids to fully heal yourself as they won't count as first aid medicines uses. A feature that comes in handy for this trophy is that your health will slightly regenerate over time and once you enter a different area, enter a save room or trigger a cutscene. This will make sure you will survive one or two more enemy hits every time it regenerates your health slightly back. This means that it's okay to take damage if you know what you are doing and that it doesn't instantly mean you will have to heal yourself with a first aid medicine.
One last thing that you might want to keep in mind here is that you can choose to pick the other ending to get that trophy out of the way as well. This also makes sense since it will allow you to skip a mini boss fight on your Madhouse run.
Stage 3: Madhouse Difficulty Playthrough and All Antique Coins
All that is left now is your Madhouse difficulty run. While this setting can be really challenging at times, you should be really well-prepared by now. Provided that you followed this roadmap you will have obtained a couple of crucial items which will make this difficulty much less daunting: The Essence of Defence together with The Secrets of Defence will reduce the damage you take while blocking to almost zero, the Circular Saw will help you deal with most bosses in minutes without having to worry about ammunition. This being said, the worst part of your Madhouse playthrough might well be the Mia bossfight in the attic, since you will have to fight her without those precious items. There are a lot of minor and major differences when playing on Madhouse difficulty: The enemies hit harder and take more damage, they are also placed differently so watch out for fresh jump scares. Saves are limited, since you'll need cassette tapes in order to save. There is not shortage of those tapes though, and they can be found in plain sight next to the cassette recorders. If you collect only those which are in plain sight you will be easily able to save about a dozen times, so you can basically save whenever you come across a recorder. Another big difference has to do with the Antique Coins. There are 33 of them on Madhouse difficulty, while easy and normal had only 18, and they are placed differently. Getting them is not only worth it because of the trophy, they will also unlock juicy items which will make your life on Madhouse much easier.
Tips & Strategies
- You can press in the menu to switch to the combine tab; this saves time and makes it easy to see which items you can currently create
- Psychostimulants are very useful, use them once an area is cleared to scavenge any leftover items
- Enemies cannot follow you everywhere, there are certain boundaries to their movement, like doors to different areas; use this to your advantage
- X-Ray glasses are cool, especially on Madhouse. Carry them if you can spare the item slot.

You will get this trophy as soon as you finish watching the first few cutscenes of the game, right after you exit your car.
At the start of the game, you see Mia recording a video for Ethan were she confesses that she lied to him. Here only message is that he needs to stay away and should not come look for her. However, in the next cutscene you see Ethan driving towards a mansion located in Dulvey, Louisiana while having a phone call with an unknown person. He claims that Mia has contacted him to come and get her, while he thought she was dead, because he hadn't heard of her for over 3 years.

You will get this trophy as soon as you beat the first boss of the game and escape from the guest house.
Once you reach the Attic in the guest house, you will have to face Mia one last time. If you try to climb up the ladder, Mia will be waiting for you with a chainsaw. A difficult fight will follow, as your ammo is very limited and the area is really small. Once you have defeated her, go into the storage room attached to the attic where an unpleasant surprise will be waiting for you. Here you will meet Jack Baker for the first time as he knocks you out from behind and welcomes you to the family.

You will be awarded this trophy upon successfully obtaining three key items which will open the front door of the main house.
The three items in question are three dog heads. The first can be found in the Living Room, right next to the room you came to after being taken from the guest house. Use the Clock Pendulum you find in the Main Hall with the Grandfather Clock in the Living Room and the White Dog's Head is yours. The Blue Dog's Head can be found in the Recreation Room upstairs. It's hidden inside a book. For the last Dog's Head you need to head down into the Processing Area. At the end of this section, you will meet Jack Baker for yet another challenging boss fight. There is a pillar in the middle of the room and there are a couple of corpses hanging from the ceiling. You can kick those corpses with to slow Jack down and make it easier to attack him. Use your shotgun to shoot him in the face until he goes for the huge chainsaw. After this, grab the smaller chainsaw for yourself and start your new career as an experimental hairdresser. Attack him until he goes down, then use and to plunge your saw deep into his brains. Do this twice (on normal) and you should be able to take him apart for now. Head back upstairs and use the last head in the front door to unlock it. Leave the main house and the trophy will unlock.

There are a lot of minor and major differences when playing on Madhouse difficulty. First of all, the enemies will hit you harder and take more damage. They also spawn in different locations, so watch out for fresh jump scares. Saves will also be limited, since you'll need cassette tapes in order to save. There is no shortage of those tapes though, and they can be found in plain sight next to the cassette recorders. The game does still make an auto save for you in a few spots, but they are very limited as well. Below is an overview off all the auto save locations on Madhouse difficulty. Be sure to keep them in mind while you are considering to make a manual save or not.
- Just before you lose your hand to Mia's chainsaw attack in the Guest House.
- After you are brought to the Main House and break out of the chair in the room where you are having dinner with the Baker family.
- After watching Lucas' video in the Testing Facility where the TV explodes.
- Right before you have to fight Jack for the third time on the boat yard, after taking the serum from Zoe.
- Right after the boat ride, where you get to play as Mia.
- When you reach Floor 1 of the Wrecked Ship, right after the cutscene where Evie runs off and says 'You said we'd be a family. YOU SAID!'.
- Right after starting the videotape sequence on the Wrecked Ship that reveals Mia's history as Eveline's caretaker.
- After exiting the elevator on floor S2 in the Wrecked Ship, still during the video tape sequence.
- Right before the only skippable cutscene in the game where the Wrecked Ship gets destroyed.
- Right before the next cutscene in the game where Jack will reveal Eveline was behind all of this and that he nor his family are killers.
- After you get to play as Ethan again, right after Mia saves you from the cocoon.
- During the section in the attic where you will have to inject the serum into Eveline, where she tries to push you away with her force waves.
- The last auto save of the game will be right after you've injected Eveline with the serum.
As you will notice, there is a massive gap between #2 and #3. Make use of your manual saves throughout this section wisely, as you most likely won't have too much trouble from that point on. There are plenty of cassette tapes in the game, and most of them are actually lying around in plain sight around the recorders. If you collect only those which are easy to find, you will be easily able to save about a dozen times. Another big difference on Madhouse difficulty has to do with the Antique Coins. There are 33 of them on Madhouse difficulty, while easy and normal had only 18, and they are placed differently. Getting them is not only worth it because of the trophy, they will also unlock juicy items which will make your life on Madhouse much easier.
Last but not least, the boss fights will also way harder than on easy or normal difficulty. However, once you have access to the first item box, things will lighten up considerably. A lot of bosses can be dispatched within minutes using the Circular Saw, same counts for most regular enemies. If you combine this with The Essence of Defence and The Secrets of Defence, beating the game on Madhouse should become manageable. More often than not you will find yourself in a situation where you are feeling safer with the door being closed behind you. Most doors in the game will close automatically for you once you get out of range, however for this trophy you will have to manually close any door yourself. You can close an open door by pressing the button on it.

Guarding is one of the more useful gameplay mechanics in the game and will ensure that you will only take a minimal amount of damage from an enemy attack. To guard an enemy attack, all you have to do is simply hold the button and you will automatically block an incoming attack.

You will come across many opportunities in the game where you can earn this trophy. The first opportunity is at the very start of the game, right before entering the guest house. You will come across a burned-out fireplace in the open with a bag lying right next to it. Take the bag and examine it closely. You will find a driver's license inside of it. The trophy will unlock once you pick up the driver's license by pressing the button.

Lock picks can be used to open desk drawers and item boxes for some additional goodies to help you on your journey through the Baker family nightmare. You will find your first lock pick in the first save room. Look out for a yellow box. Pick it up and examine it. You can open it by pressing once you have found the correct angle. Use the lock pick on any applicable lock - for example the little table in the hallway near the Living Room - and the trophy will unlock.

You can find a Shadow Plinth in a small alcove next to the main hall. Look out for a waist-high stone column illuminated by a spotlight. You are supposed to use a wooden statuette with it and align it so that its shadow matches the shape on the picture on the wall. For this trophy, however, simply try using a different object, for example the Clock Pendulum, the knife or one of the Dog’s Heads. The trophy will pop shortly after doing this.
Open Your Eyes Use psychostimulants. |

Psychostimulants is a new item in the RE universe. Using it allows you to see the locations of all collectibles for a limited amount of time. Little markers will pop up on screen and you can even see them through walls, floors and ceilings. The nearer you get to an item, the bigger the marker will become.
The first opportunity to unlock this trophy is after you have solved the puzzle involving the Wooden Statuette in the main hall. Squeeze through the new passageway and you will find Psychostimulants in the room with the deer. The trophy will pop upon usage, regardless of whether you found any items using it or not.

Once you have reached the Old House you will finally get the opportunity to increase your limited item slots. There is a backback sitting on a table and waiting for you in the save room in the northwest of the area. Simply pick it up to increase your item capacity and unlock this trophy. The easiest method to obtain this trophy is killing the big flies in the Old House. They are roughly the size of a kitten so you can't really miss them. It takes only one slash with your trusty knife to kill them; once you do, the trophy will unlock. Keep in mind that the swarming flies do not count, you need to go for the bigger ones.

You will encounter insect-covered doors in the Old House for the first time. You will notice that some of the doors (normal doors but also cabinet doors) are covered with some greyish stuff. Upon approaching those doors you will find out that you were not looking at mold, but at hundreds of tiny itsy-bitsy spiders, ready to jump at you! Eww! The normal (and most efficient) method of dealing with this problem would be to burn them with fire. However, for this trophy you'll want to use the knife and keep slashing until they are all gone. Don't worry, they don't deal a huge amount of damage and it doesn't take too long to get rid of them. Once the door is cleared the trophy is yours.

This trophy probably will not come naturally since it requires you to waste a lot of ammunition on an enemy you do not have to fight. Once you have reached the Old House you will soon meet Marguerite Baker. Do not worry, the encounter is story-related and you cannot miss her. She will be wandering through the house and it is actually pretty easy to avoid her, since she is carrying a lamp which makes it easy to see her coming. For this trophy, however, you need to pump lead into her until she runs away. Make sure to build the Flamethrower before attempting this to make fighting off the insect swarms she sends at you easier. Otherwise just keep attacking her until she has had enough. As with all enemies, headshots do way more damage, so go for those. Once she pushes you to the ground to run away, the trophy is yours.

This trophy can only be obtained during the Jack boss fight in the Processing Area. Once you have dealt enough damage, Jack will have enough of you and go for a massive scissor-like double chainsaw. One of his attacks with this weapon is a charging attack. Jack will be coming at you with open scissors, trying to cut you in half. If successful, this can be a one hit kill, so be careful. To avoid the attack and get this trophy, simply press at the right moment to duck. Jack will miss and the trophy will unlock.
Less is More Take down two or more enemies with one shot. |

This trophy can easily be obtained in the Old House, were you will be introduced to Insect Swarms. Let one of those swarms get near you, then simply use your shotgun to blast right into it. Not the most efficient way to deal with insect swarms, but certainly the way to go to get this trophy.

This trophy can only be obtained during the boss fight against Marguerite in the greenhouse. It is recommended to create a manual save file before going for this, in case you kill her before unlocking the trophy.
Start the fight and hit her a couple of times. Once you have dealt some damage to her, she will start climbing the ceilings and walls. Keep a close eye on her since she is about to jump at you. She will stop crawling and look at you before leaping off, so watch out for that. Use your shotgun and fire at her shortly after she jumped. You need to hit her in mid-air for the trophy to unlock. If you did not get the timing right, wait for her to climb the walls and try again. If you need some visual aids refer to the video below:

Remote Bombs will become available on the ship during the later phase of the game. They are extremely powerful and allow for strategic enemy disposal. You can place them on the ground with and detonate them with . For this trophy, however, you need an enemy to grab you while having a remote bomb equipped. You will be prompted to attach the bomb to the enemy. The bomb will go off, saving your ass and unlocking another trophy.

Chem Fluid - an item vaguely resembling an urine bag - and Strong Chem Fluid (same in red) can be used to create a number of new items, ranging from healing items to ammunition. To combine them, select the Fluid and choose 'Combine with' or simply press while you are in the menu to switch to the combine tab. Below you can find a table presenting all possible combinations. Once you have successfully created at least one of each the trophy will unlock.

You will find the "Mia" videotape in the Recreation Room on the second floor of the Old Mansion. Conveniently enough there is a TV and a VCR in the same room, so you can go for this trophy right away.
You will be playing as Mia and you must not be spotted by Marguerite to unlock this trophy. Avoiding her is not terribly hard; simply hide every time you can hear her come nearer. You can use the boxes at the beginning of the area and wait until she has left through the closed door, this will give you some time to sneak through the area. If she returns you can hide outside, just around the corner. You will be able to see her rummaging around inside through the cracks in the wall, so you'll know when it is safe to go back inside again. If Marguerite should see you simply wait for her to grab you, this will trigger a checkpoint restart, allowing you to try again. Of course you can replay the video on any VCR in the game whenever you like. Below is a video guide if you prefer visual aids.

The "Happy Birthday" videotape can be found in the cabinet in the attic of the Old House, in the room where you obtain the Red Key Card. The good news is that you do not have to complete the tape within 5 minutes on your first attempt. It will remain in your inventory and you can watch it again and again on any VCR in the game (e.g. the one in the trailer). This also means that you do not have to use the telescope to find out the combination for the medicine cabinet, since it will not change. Simply follow the step-by-step instructions below or watch the video and you should not have any issues with the trophy requirements.
Grab the Candle.
Go into the room with the Birthday Cake, this will trigger the actual "game".
Open the medicine cabinet (combination Chainsaw Man, Hat with Raven, Baby, found by getting Telescope out of Toilet and cleaning it via Cake room entrance, then using it on monitor wall)
Get the Winding Key from the keg
Leave the room and re-light the candle using the stove.
Burn the Straw Doll using the stove to get the Dummy Finger
Burn the rope on the door with the candle.
Grab the Balloon from the next room
Use the Balloon with the leaky pipe to get the Quill Pen
Use the Dummy Finger and the Quill Pen with the left hand of the guy who had the candle, then use the Winding Key in his torso to activate him
Enter the word Loser in the lock of the door in the room with the balloons.
Grab the Valve.
Use the Valve with the water pipe in the cake room.
Re-light the Candle on the stove.
Put the Candle on the cake.

There are 33 Antique Coins on Madhouse difficulty, while easy and normal had only 18, and they are placed differently. Getting them is not only worth it because of the trophy, they will also unlock juicy items which will make your life on Madhouse much easier. Four hours is a very generous time frame for a Resident Evil 7 speedrun, so it is recommended to combine this trophy with Resource Manager - don't open the item box more than three times - and Walk it Off - only use three First Aid Meds or less.
A speedrun time of about 2 - 3 hours is perfectly possible on your second playthrough, especially if you are referring to a video walkthrough whenever you are lost. Use to pause the timer while doing this! Dying or restarting checkpoints manually will add to your time, so it is recommended to create multiple manual save files you can fall back to if the shit hits the fan and you have to replay a certain section. To check your time, press and go to Stats. Here are some reference times to help you evaluate your progress. The times are very lenient, but as you can see from the final time you could even be a tad slower and it would still be in the trophy time frame.

The most important point about this trophy is that the game forces you to use the Item Box three times! This means you are not allowed to use it at all apart from those occasions if you want to obtain this trophy. There is a workaround for the third time you are forced to use the box, so technically it might be okay to use it once in between. However, it's actually not necessary to do that since this trophy is not that hard at all.
First off, you should obviously be playing on easy when attempting this. As stated above, it's sensible to combine this trophy with Just Get Me Outta Here and Walk it Off. Since you will be throwing away most of your healing items you shouldn't have too many issues with inventory space. If you were following the roadmap you shouldn't be forced to pick up and Coins, either. The tree times you are forced to use the Item Box are before and after the 'Birthday Game' and after escaping from the Old Ship.
If you die during certain boss battles, you will be presented with a 'Manage Items' option. This does not count towards this trophy, so technically you could skip the Item Box after the Old Ship and get your weapons through this option. However, this should only be necessary if you somehow managed to open an Item Box by accident. If you want to check on that, hit and select stats.

This trophy is way easier than it sounds. It can (and obviously should) be done on easy and can be combined with Just Get Me Outta Here and Resource Manager. The good news is that you can actually 'walk damage off', you will regenerate a small section of health, so you won't be at the brink of death forever after screwing up once. You are allowed to heal three times, so you can save those for boss fights if you like. It is recommended not to pick up healing items or to throw them away so you won't accidentally use them. If you want to check on that, hit and select stats. Steroids don't count and can be used; they will heal you completely and increase your total health permanently, giving you some additional healing chances.
